IOLS MegaSPEED - Service Request Form

IOLS MegaSPEED service is available to current and new IOLS Customers only and is billed to your existing credit card information on file at IOLS.
 
Simply complete the form below, submit it, and start your download and installation of the IOLS MegaSPEED software.  The service will be activated once we have billed your credit card (Usually within 8 hours of your submission).  Upon activation we will also send you notification via email.
 
The additional cost of the service is as follows:
   - Monthly (Month to Month) $7.00 per month.
   - You may also to choose to pay for the service per the term
      of your account with us (Quarterly or Annual only).
     The additional cost to do this is $5.00 per month billed per your term*.
      * The Initial charge will be billed for the remaining months in your current term.
          Following the initial charge you will be billed the full amount per the term in addition
          to your IOLS Dialup Internet Access Account.
 
Note:  You must have a current IOLS dialup account to use this free trial.  If you do not have an account with IOLS please click here and activate a dialup account today.

NOTE: All fields Are Required Full Name IOLS Username IOLS Password Password must match what is on file. IOLS Email Address Billing Term All charges are prepaid and are in addition to your current charges with IOLS. Month to month ($7.00/mo.) Billing is month to month at $7.00/mo. prepaid each month in addition to your current charges. Per my agreement term ($5.00/mo.) Billing is per your current agreement term (Quarterly or Annual Only) at $5.00/mo. prepaid for the number of months of each term in addition to your current charges. Agreement I AGREE to the following (You must agree to this for the service): I agree to pay the additional charges for the IOLS MegaSPEED service per the billing term above with the credit card information I have on file at IOLS. I also understand that I may cancel the IOLS MegaSPEED Service at any time and keep my IOLS Dialup Account.   I do not agree to the agreement.   Upon submitting the form you will receive instructions for downloading and installing the IOLS MegaSPEED software.
